Scheduling an Appointment Is This Easy

We intend to make seeing our doctors convenient for you. But you need to do your part, too. So when you call, please tell our appointment secretary why you need to be seen, and if we've seen you before. Just describe your symptoms as best -and straightforward-as you can. No need to be shy with us. In fact, it is important that you be blunt.
Of course, we'll have a few questions about your insurance plan, just to speed things along once you're here. Be prepared with your insurance card on your first visit. Naturally, we'll have paperwork to take care of, but we're very sensitive to your time schedule. So we've built in plenty of time for that in setting up your appointment.

Please bring your Insurance cards with you, Payment is requested for services rendered at the time of your visit.


We would like to emphasize that we are a Urological Medical-Surgical Group specialty and, as such, are involved with surgical emergencies. If this should occur and involve a need to change your appointment, you will be notified at the earliest possible time.

For New Patients:
We would appreciate it if you could download and complete the New Patient Registration Form and Genitourinary Exam Form and bring them with you the day of your visit as well as a list of any medications you are taking.
